A completed registration form is required for each registrant. Registrations are processed on a first-come, first-serve basis. Before you sign on this form, please confirm information on pricing, course location and course description. For any other information, contact Unidustry at firstname.lastname@example.org. Incomplete or unsigned registration forms will not be processed and may delay class reservations. You will receive an acknowledgment letter notifying you of
receipt of payment and registration status. Classes are normally confirmed or cancelled based on enrollment approximately 10 business days before the scheduled start date.

To expedite registration, please follow these directions:

c. Payment must be made 5 working days prior to the scheduled training date.
It is sometimes necessary to change the dates of a class or to cancel a class. Registrants are contacted at the earliest opportunity in the event of a scheduling change or cancellation.
Transportation to the training site is the registrant’s responsibility. Unidustry assumes no responsibility for nonrefundable travel arrangement losses resulting from course scheduling changes or cancellations.
a. No refund is allowed if the trainee cancels the course registration, or no show, or attended only a part of the course.
b. No refund is allowed if the trainee provided an invalid student ID upon registration, or student ID expired after registration and lead to being banned from attending the course.
c. No refund is allowed in the event of a scheduling change.
d. Refund is allowed if the scheduled course is cancelled by Unidustry and no immediate rescheduling within 30 days from the initial date.
e. Refund notification will be sent to the trainee by email and the amount will be refunded to the payee account within 30 days.
Registrants will be provided a class location and a list of area hotels. Registrants are responsible for making their own lodging arrangements. Meals are not provided.
a. All course materials (document) are provided at the training site in soft copy only.
b. If consumable is required, it is included in the course fee with limited supply only.
Certificate of competency can only be issued upon trainee completed the course and passed the test and project given
at the end of the course.
A mentoring fee of RM500/trainee is included in the fee. Mentoring by trainer shall be total three (3) times in the
period of 2 months from the date of training in group basis only. Split or Individual mentoring session is not an option.
